I met him, fifteen years ago. I was told there was nothing left. No reason, no conscience, no understanding; even the most rudimentary sense of life or death, good or evil, right or wrong. I met this 6-year-old child with this blank, pale, emotionless face, and the blackest eyes; the devil's eyes. I spent eight years trying to reach him, and then another seven trying to keep him locked up because I realized what was living behind that boy's eyes was purely and simply… evil.
~ Dr. Samuel Loomis contemplating his former patient Michael Myers.
I believe he is quite sane. Just evil.
~ Dr. Stephen Kellerman describing Scarecrow.

Psychopaths/Sociopaths are Pure Evil villains suffering from psychopathy, also called sociopathy or have all the symptoms of such - this includes an inability to feel empathy, superficial charm, a tendency towards violence (in some individuals), narcissistic traits, extreme reactions to rejection or criticism and a habitual contempt for other people's rights. Pure Evil Psychopaths are amongst the worst of all pure evils because they are unrepentantly cruel and ruthless towards everyone and everything, completely devoid of any humanity and all care for anyone but themselves, although sometimes they may be so callous that they do not even care about themselves due to impulsivity and lack of regard for own safety and life (e.g. William Lewis, Jim Moriarty and Bryan Fury). Many of them are sadists, but their cruelty doesn't always manifest itself in sadism, and it's very possible for them to feel nothing at all about the atrocities they commit.

Sympathetic Pure Goods are their good opposites.

Fake Tragic Pure Evils usually fall under this category due to the fact that they cannot be sympathized with, but exceptions exist if they have other mental illnesses.

Psychopaths are similar, but distinct to sociopaths.

  • While psychopaths can hide, control disguise their antisocial traits under a superficial façade, are able to educated and blend in with society if necessary, a sociopath is much less able to control and disguise their antisocial outbursts due to a tendency to rage and lack of restraint thus the common saying goes: "cold-blooded psychopath, hot-headed sociopath".
  • Thus psychopaths tend to be very successful individuals who seek positions of power and often manage to do so, while sociopaths are more prone to disorganized lives filled with impulsive crimes, reckless behavior and general hatred for "social norms".
  • It is also believed that psychopaths are born with the condition from birth, while sociopaths are formed via repeated traumas, neither are considered insane and thus no "cure" exists for either condition (although it is treatable), though some individuals may become less violent with age (though by no means all).

NOTE: Despite media often confusing the two, psychopathy and sociopathy is NOT the same as psychosis. While psychotic characters suffer from extreme mental illnesses that cause them to lose control of their behaviour, thoughts, emotions and dissociate from reality in various ways; psychopaths and sociopaths, on the other hand, are rational, coherent and sane at all times and although they may have twisted views of reality and be threat to society and other people, they fully understand how moral and ethical rules work, they simply don’t care to follow them (thus under law both psychopaths and sociopaths are treated as legally sane, even the hot-headed sociopaths [lack of control over one's temper is not sufficient to be considered a detached from reality, they simply abstain to restrain their emotional instability].

Also not all Pure Evils are automatically antisocial, even though most of them have psychopathic traits. Some may not do most of the actions within a sociopathic/psychopathic mindset, or suffer from mental illness or psychosis. Examples of Pure Evil villains that isn't antisocial, but still meet the Pure Evil criteria despite not meeting most of the psychopathic traits is Demise, Black Doom, and The Demon King.
