Pure Evils who suffer from dissociative identity disorder. Most well known for having split personalities.

Under normal circumstances, Dissociative villains rarely qualify as Pure Evil, given they have no control over their actions due to the disorder and they don't remember anything their other personalities did, but some of them may qualify if they have a clear moral agency of the crimes they've committed and had since grown accustomed to this.
